Service Line Coordinator Hospice

32 hrs/wk; 4 days/wk plus call requirement. RN responsible for monitoring and reporting quality metrics and patient satisfaction. Completing chart audits. Coordinate CMS and The Joint Commission requirements and updates and assist with readiness for surveys. Manages biweekly IDTmeetings. Assist with referral management and triaging phone calls. Clinical duties include wound care and vacs, IV antibiotics and TPN, head to toe assessments, patient and family education in patient’s home/residence. Will provide direct patient care 2 days/week and when on call and will perform SLC duties 2 days/wk. Will also provide clinical support for field staff, patient scheduling, other managerial duties and audits. Must be able to work independently. Excellent communication and customer service skills, strong computer skills, team player with previous nursing experience needed. BSN or willing to obtain, NE RN license, BLS, Able to drive own vehicle, valid drivers license and auto insurance. Prefer candidates with home health, hospice, case management experience.
